Job description

You will translate policy into operational practice across the community care landscape, supporting care coordination and clients’ care transitions.

  • Translate policy into operations: develop clear workflows and implementation plans, working with stakeholders across AIC, MOH, and service providers to ensure aligned, consistent, and effective execution across community care services
  • Support appeals and referral operations: ensure timely, appropriate, and consistent decision-making, supporting fair access to services and smooth care transitions for clients
  • Monitor operational performance and drive improvements: track referral performance, centre utilisation, and waitlists, and provide actionable insights to identify gaps, reduce delays, and improve overall system efficiency
  • Support national initiatives and care integration efforts to strengthen care coordination and enable more seamless transitions across care settings
  • Drive systems and process improvements by collaborating with stakeholders to implement system enhancements and streamline workflows, improving referral efficiency and user experience

Job Requirements:

  • Professional qualification in Nursing, Social Work, Allied Health disciplines or a similar field along with relevant healthcare experience
  • At least 3 years of experience in related field in acute or community healthcare setting, or community care setting in Singapore
  • Knowledge in geriatrics, care coordination, community services, and customer service would be an advantage
  • Able to work independently and collaboratively, manage multiple priorities under pressure, and adapt to the evolving complexities of the community care system.
  • Strong analytical, critical thinking, and problem-solving skills
  • Strong written and verbal communication, organisational, and interpersonal skills.
  • Effective stakeholder management across clients, service providers, Ministry, and internal divisions
  • Proficient in Microsoft Office (e.g. Excel, PowerPoint, Word)
