Why This Role Matters

Delivering high-quality care across the continuum requires strong coordination between clinical operations and community-based programmes. This role supports the Division of Medicine in enhancing clinical workflows and service delivery, while contributing to the NHG DEFINITE Programme—an integrated, multidisciplinary initiative aimed at improving care for patients with diabetic foot ulcers and reducing the risk of amputations.

By working closely with clinicians, researchers, and community partners, you will help strengthen care continuity, support programme development, and drive data-informed improvements to patient care. This role is ideal for individuals who are passionate about healthcare operations, integrated care, and improving patient outcomes.

What You Will Drive

1. Clinical Operations & Workflow Improvement

  • Support the execution and improvement of clinical and non‑clinical workflows across Division of Medicine services.
  • Collaborate with clinicians and stakeholders to develop and implement improvement projects for care pathways, services, and programmes.
  • Facilitate discussions and reviews by documenting meeting minutes and ensuring timely follow‑up and closure of action items.
  • Extract, monitor, and report key performance indicators for internal review and submission to MOH.
  • Liaise with clinical departments to coordinate data requests and operational requirements.

2. Procurement & Administrative Support

  • Provide administrative and secretariat support for procurement activities related to medical and non‑medical equipment.
  • Support initiatives such as equipment standardisation and integration into operational workflows.
  • Prepare proposal papers and documentation to support approval processes (e.g. pre‑sourcing, evaluation, and award).
  • Track project timelines and ensure timely follow‑up and closure of key deliverables.

3. DEFINITE Programme Coordination & Patient Care Support

  • Support patient care coordination across inpatient, outpatient, and primary care settings for patients with diabetic foot conditions.
  • Perform patient assessments, documentation, monitoring, and follow‑up throughout the continuum of care.
  • Provide education and counselling to patients and caregivers, including diabetic foot care and health coaching.
  • Assist in diabetic foot screening and support podiatry services where required.

4. Programme Development, Data & Research Support

  • Support the development, implementation, and evaluation of programme strategies, workflows, and care models.
  • Collect, analyse, and manage data for programme monitoring and reporting.
  • Support quality improvement and research initiatives, including coordination of studies, data collection, and patient enrolment into programmes and registries.
  • Work with clinicians, research teams, and programme staff to ensure effective execution of research and programme activities.

5. Administrative & Secretariat Support

  • Provide administrative support for programme and departmental activities, including meeting coordination, documentation, and reporting.
