Education and Professional Certifications/Licenses

• Bachelor degree in Social Science or Arts Degree in Social Work with no experience required
• SCHS Registration/License

Job Purpose

To provide social work services for patients and their families in the resolution of problems arising from their medical conditions and circumstances.

Key Accountabilities & Responsibilities

• Visits the Wards/Units regularly and establishes professional relationships with the medical and nursing staff and ensures that the treating physicians and nurses are fully aware of the social circumstances of the patient.
• Interviews patients, family members and friends to develop background knowledge concerning the patients’ social life in order to assess their social needs.
• Prepares and implements an appropriate management plan to alleviate adverse effects upon patients; ensuring maximum utilisation of all available resources to achieve this goal.
• Educates the patient and family in the understanding of and adherence to the medical and social management plans.
• Trains and shares knowledge with other staff and students
• Refer patients to social welfare agencies after reviewing their cases.
• Assists in solving patients’ problems, which hinder their treatment, such as providing financial, social and physiological support.
• Refer the patient to the Ministry of social devlpoment to get the disability benifts for individuals with special needs. (car Parking Card , Airfare discount card ) and the Financial Aid and rehab equipments
• Prepares necessary reports and statistics supported by feedback/ remarks, as necessary, and submit such reports to the Social Work Supervisor for necessary actions.
• Provide home on-call for emergency cases (Abuse-Neglect, Suicidal attempt and Social &family issues)
• Assess the psychosocial needs of patients and their families in the clinic, considering factors like socioeconomic status, mental health, family dynamics, and support systems and provide emotional support and counseling to patients and families coping with illness, trauma, or loss.
• Respects patients and their families to promote for a Patient-Centered Care culture.
• Participates in and supports quality improvement and patient safety activities as an individual or as part of a multidisciplinary team.
• Perform other job related duties as assigned.
